Application Form for applicants to the the BCHS program

It is crucial that you send official transcripts
for all colleges you previously attended to the same address as your $20 fee
(NMSU Admissions Office, MSC 3A, P.O. Box 30001, 88003-8001).

A $20 undergraduate application (nonrefundable) fee will be assessed upon receiving this application.

Important note: Students who apply online are still responsible for the $20 application fee. The Admissions Office will not begin processing applications until the fee is received. To pay the fee, please send a check or money order for $20 to NMSU Admissions Office, MSC 3A, P.O. Box 30001, 88003-8001.

The student's name must appear in the memo line of the check, so that checks may easily be filed with each application.

If you would like to pay using a credit card, please call the Business Office at 505.646.4911, and if you are using someone else's credit card please make sure the Business Office representative knows the application for which the fee is being paid.

The BHCS program currently is not appropriate for first-time freshmen. Required general education courses may not all be available yet via distance education at NMSU. First-time freshmen interested in the BHCS program should contact the College of Health and Social Services at (505) 646-3526.
