INSTRUCTIONS:
This form is for College of Health & Social Services (CHSS) Research Center internal use. The objective is to provide advance notice to the college Assistant Director in the Dean’s office of competitive research, instruction or public service announcements that may be pursued by faculty or staff.

The form can be filled out and e-mailed to Ancelmo "Sam" Encinias, CHSS Research Center Assistant Director.

This advance notice will allow for “up-front” assistance and support from the CHSS Research Center Assistant Director. This individual will perform a quick review of the RFP, RFA, or other program solicitation in order to identify and flag any potential administrative or financial requirements that may be difficult to comply with.
