    Directions for Scholarship Application Completion:
    1. Open or download the application form (PDF - requires at least Adobe Acrobat Reader v6 or newer)
    2. The form is not "fillable" at this time; you must first print out the packet. Please print legibly.
    3. Follow the directions on every page.
    4. Please print clearly, especially your email address, Banner I.D. number, and other pertinent information requested.
    5. Examine carefully your essay for correct grammar and sentence structure. Have someone else edit your work.
    6. Note the ultimate due date of Monday, March 3rd, 2008. Please return completed hard copy of your application packet to Dean's Office of the College of Health & Social Services, Room 310 in the Health and Social Services Building.
