AIP Scholarship form

• Current semester Registration Document (Banner print outs are acceptable)
  
• A typed paper statement of your demonstrated financial need
• A typed paper statement of participation (or planned participation) in the American Indian Program and/or its student organizations/activities, Peer Mentor Program or FAIS Program
• Copy of previous semester grades (not required for new students)
• Certificate of Indian blood (CIB)
• A signed Tribal Information Sheet

• Fill out Application
• Submit all required information by due date
• Half of the awards will go to those with demonstrated financial need and half will be merit scholarships to students with a 2.50 or better

Note: If the number of qualified applicants exceeds available scholarship funds, awards will be determined by the submission date of the complete application package. Awards will be pro-rated based on the number of students enrolled. The amount of award will be subject to the availability of funding and the number of applicants.
