The New Mexico Dept. of Health, Public Health Division, Border Health Office is
issuing this Request for Proposals (RFP) to conduct an Environmental Health Outreach
Program by conducting assessments 0 home environments from an environmental health
standpoint for the communities of Chaparral, Sunland Park, and La Mesa.

Proposals are sought from eligible applicants meeting the following requirements:
For-profit and non-profit community based organizations, companies, educational
institutions, and governmental entities, including tribal organizations, having conducted
previous environmental and/or community health outreach. All interested applicants
must attend the proposal pre-submission conference in order to be eligible for funding.
The pre-submission conference will be held on Tuesday, August 31, 1999 at the Border
Health Office conference room from 1:00 p.m. to 3:00 p.m. located at 1170 N. Solano
Drive, Las Cruces, New Mexico.

Contracts may be awarded for the period beginning with the date of approval by the Dept.
of Finance and Administration through June 30, 2000 with possible renewal of contract
for a period of up to, but not exceeding, three years from date of contract signature,
contingent on future funding as determined by the Division and satisfactory performance
of contract Scope of Work.
