Incentive Retirement Program
Deadlines
and Documentation Requirements
Deadlines and Documentation Requirements:
- November 30, 2005: Employees must complete and return
a Request for ERB Retirement Estimate Form to the Employee Benefits Office
no later than November 30, 2005. (Note: Requesting a retirement estimate does
not constitute an obligation to retire.)
- March 1, 2006: An Incentive Retirement Program Form must
be completed and signed by the employee, the immediate supervisor, the Dean/VP
and the Director of Human Resources as soon as possible but no later than
March 1, 2006. The form may be obtained from the Employee Benefits Office
by making an appointment.
- March 1, 2006: Written notification of an employee’s
decision to retire must be submitted to the immediate supervisor and the Dean/VP
with a copy to the Employee Benefits Office as soon as possible but no later
than March 1, 2006.
- April 1, 2006: All retirement paperwork must be completed
and submitted to the Employee Benefits Office no later than April 1, 2006.
To complete paperwork an appointment must be made with the Employee Benefits
Office.
- July 1, 2006: Employees must retire effective July 1, 2006.
In very rare cases and under extraordinary circumstances, the Provost may
grant an exception for an employee to retire prior to July 1, 2006. Requests
for exceptions must be recommended by the Dean/VP and approved by the Provost.
Under no circumstance will an employee retire after July 1, 2006 under this
program.
Employees with Retirement Paperwork Completed as of October 17, 2005:
Regular employees who meet the eligibility requirements and who have completed
retirement paperwork as of October 17, 2005 will be eligible to participate
in the program with a retirement date prior to July 1, 2006. An Incentive Retirement
Program Form must be completed and submitted to the Employee Benefits Office
no later than the last day of employment or November 15, 2005, whichever is
earlier.
Please note that all deadlines
are firm because of the anticipated high volume of requests to retire.
