Qualifications: - Bachelor’s degree and one (1) year of experience or any equivalent combination of education and experience which provides the required knowledge, skills and abilities.
- Must possess high degree of people, problem solving and interactive skills.
Examples of Duties: - Make assessments and place individuals in work activities.
- Establish and maintain a case file, establish and review an Individual Responsibility Plan (IRP), provide intensive and comprehensive direct case management that helps participants engage in treatment for barriers that impede their success in training and/or employment.
- Track participants’ compliance with treatment and work activities by maintaining regular client contact and conducting home visits.
- Arrange for further assessments such as psycho-educational testing for learning disabilities.
- Refer participants to treatment/service providers appropriately.
- Ensure that participants engage in and continue treatment for the time period specified by the treatment providers; schedule meetings to review participants’ progress.
- Maintain the case file and data entry requirements.
- Conference with the supervisor to determine when participants are ready to begin job training, job search, employment or other work activity and assist them in making that transition.
- Performs other related duties as assigned.
