If registering by mail, please include your full name, social security number, mailing address, telephone number and e-mail address. Please indicate if this is your first time taking the Miller Exam. If it is not, indicate the number in the bottom right corner of your retest admission ticket.
ID requirements: On the test day, the candidate mus present two (2) forms of ID. One must be a government or school-issued photo ID. The second can be a non-photo bearing ID (such as a social security number card or birth certificate.)
REFUND POLICY - Any refund requires your original receipt and your test ticket. Rescheduled tests do not qualify for a refund. The business office will mail your refund check in approximately three (3) weeks. There are NO exceptions to this policy. Refunds will be made as indicated below:
Seventy-five (75%) test fee refund if you cancel a scheduled test and request a refund in writing at least 14 days before the scheduled test
Twenty-five (25%) test fee refund if you cancel a scheduled test and request a refund in writing by at least 2:00 p.m. on the last business day before your scheduled test.
