New Certification

We are now accepting forms for SU MS2 & FALL '09. Please submit the following.

IF YOU APPLIED FOR POST 9/11 - CH33, FORMS WILL BE ACCEPTED AT A LATER DATE.

FORMS REQUIRED FOR COMPLETION OF CERTIFICATION:

VOE - must be the most current form (click here to get a copy).

  • Handwritten forms are not accepted
  • A form must be completed for each term
  • Select appropriate term
  • If applicable, list Community College Courses on right side under Community College Courses (DE courses are Main Campus courses and should be listed on the left side)
  • Initial statement that is listed above the selection of the terms
  • Sign form
To print a DETAILED CLASS SCHEDULE:
  • Log into MY.NMSU.EDU
  • Click from Home Tab to Student & Financial Aid Tab
  • Open Banner Self Service Folder
  • Open Student Folder
  • Open Registration Folder
  • Open Student Detail Schedule (7th folder down)
  • Select appropriate term
  • Print

Reminders:

If you are Changing your degree program, Forms 22-1995 & 22-5495 are no longer required. A degree check under your new degree program will need to be requested and submitted to our office before I can process your certification(s). You can obtain the "Request for Degree Check" form from our office.
